Important Info about Medical Forms:
All staff members should fill out and sign the appropriate medical forms as follows:
New staff
- Born before 1/1/57 – medical form can be filled out and signed by the staff member or a physician. No dates of measles inoculations are required. Current measles immunity is assumed.
- Born between 1/1/57 and 6/28/93 – medical form can be filled out and signed by a physician, parent or guardian. However, verification of the dates of your two measles inoculations must be signed by a physician.
- Born after 6/28/93 – Working papers are a MUST
Returning staff
We assume a record of measles inoculations or
measles immunity is in our file:
- Born before 6/28/93 – medical form can be filled out and signed by physician or staff member
- Born after 6/29/93 – Working papers are a MUST
- ALL MEDICAL FORMS MUST BE COMPLETED AND RETURNED BEFORE THE FIRST DAY OF CAMP.
