Important Info about Medical Forms:

All staff members should fill out and sign the appropriate medical forms as follow:

For a new staff member:

  1. Born before 1/1/58 – medical form can be filled out and signed by the staff member or a physician. No dates of measles inoculations are required. Current measles immunity is assumed.
  2. Born between 1/1/58 and 6/28/94 – medical form can be filled out and signed by a physician, parent or guardian. However, verification of the dates of your two measles inoculations must be signed by a physician.
  3. Born after 6/26/94 – Working papers are a MUST

For a returning staff member, we assume a record of measles inoculations  or

Assumed measles immunity is in our file:

  1. Born before 6/26/94 – medical form can be filled out and signed by physician or staff member
  2. Born after 6/26/94 – Working papers are a MUST

ALL MEDICAL FORMS MUST BE COMPLETED AND RETURNED BEFORE THE FIRST DAY OF CAMP.
